INTRODUCTION

IMO-Instituto de Microcirurgia Ocular (IMO), is responsible for ensuring that this notice is made available to data subjects prior to data collection and further processing of data.

All IMO employees and service providers who interact with data subjects are responsible for ensuring that this notice gets their attention.
All employees and service providers and subcontractors of IMO are bound by professional secrecy, where it is included the personal data of data subjects.

INFORMATION GATHERING

The IMO is the entity responsible for the processing of personal data, assuming the commitment that it will be carried out in accordance with the Law. The IMO collects personal information of compulsory and optional nature, using as a means the application form for marking and contact request and spontaneous application accessible through the website www.imo.pt, within the scope of legitimate interests and pre-contractual procedures.

Due to the need to comply with the service contract and compliance with legal commitments, there are several circumstances in which a user’s personal data is requested for IMO treatment, and IMO cannot confirm the performance of the pre-contractual service if the user chooses to deny the required data.

INFORMATION WE REQUEST

“Personal data” is the concept defined by the data protection law and refers to the information relating to an identified or identifiable individual.
For the purpose of making an appointment request, we collect the following personal data:

  • Identification data: Name
  • Contact Information: Email Address and Phone
  • Marking Data: Time and Doctor
  • Other: Reason for consultation

For the purpose of the contact request process, we collect the following personal information:

  • Identification data: Name
  • Contact Information: Email Address and Phone
  • Other: Message

For the purpose of processing your spontaneous application for a future recruitment process, we collect the following personal data:

  • Identification data: Name
  • Contact Information: Email Address and Phone
  • Others: Position for which you are applying, a file containing Cover Letter and a file containing the Curriculum Vitae

Personal data of minors and special category data may be sent by the user, in the course of providing our services, namely for pre-marking and spontaneous application. The information provided through the website does not allow us to confirm, taking into account the technology provided, the consent of special category data and/or consent of juvenile offenders not determine the user’s age that is providing the data. We cannot be held liable for the lawfulness of the processing of personal data provided by people who commit fraud on their identity.
IMO does not intend, in any way, to process information from minors under the age of 16.

HOW WE USE YOUR PERSONAL INFORMATION

To access the website www.imo.pt it is not necessary to make any registration. However if you need to make a “query appointment request”, a “contact request” or a “spontaneous application”, we need your registration to collect the personal information necessary for the management of the same, such as identification data and contacts. By asking to get in touch with you, you will always have the option to receive other information that may be of interest to you.

Your personal data will be kept by IMO, during the period necessary for the purpose communicated in this notice, being deleted when you cancel the order or in case of exercising the rights of opposition and limitation. After the respective storage period has elapsed, IMO will delete or anonymize data when they are not to be retained for a distinct purpose which may replace.

YOUR RIGHTS RELATING TO YOUR PERSONAL DATA

We would like you, to report any changes on the information we have about you, so that we can have correct and current information, in our systems.

At any time and when applicable to the processing we make, you may exercise your rights in relation to your personal data:

  • to obtain a copy of your personal data, together with information on how and on what basis personal data are processed;
  • to correct inaccurate personal data;
  • to erase or limit your personal data in limited circumstances, when they are no longer necessary for the purposes for which they were collected or processed;
  • to limit the processing of your personal data;
  • to oppose automated individual decisions.

In addition to the above and in case the company was not competent to resolve the complaints presented, you have the right to submit such a complaint to the competent supervisory – National Commission for Data Protection.

SHARING INFORMATION AND DATA TRANSFER

By rule, IMO do not transfer or share with third parties information relating to personal data, unless this is required by legal or regulatory requirements.

In this process, personal information can be transferred, archived or handled by site management companies, computer services, technical assistance and partners (agreements and conventions). The information may be processed in other country than the one in which it was collected and of the origin of the data subjects. In such cases, the data transfer is made in accordance with the requirements of the General Regulation Data Protection. When the transfer of personal data is made to a country outside the Union, adequate safeguards will always be provided by means of binding rules and legal force, like standard clauses approved by the Commission.
